If your insurance company is not listed or if you are unsure if your insurance company will cover your eye exam with Dr. Shettle, please call us.

If you are covered by an HMO which requires prior authorization, you will need to contact your primary care physician before your eye exam to obtain a current insurance referral. Referrals are required at the time of service. Patients who receive care without a referral may be responsible for payment at the time of service.

The insurance companies listed above does not guarantee a specific coverage. To verify eligibility of coverage, you should contact your insurance carrier. Please bring your current insurance card and information available to present at the time of your appointment.

Can I use my medical insurance for an eye exam?

If you have a medical diagnosis such as cataracts, glaucoma, macular degeneration or dry eye syndrome for example, we can fill a medial claim.

How do I know if I have Vision Coverage?

We recommend you first look at your health insurance card to see if vision is listed on your card. You may also call your insurance company and ask them if you have vision coverage. If you do have vision coverage, you will then need to determine if your vision plan is administered by your medical insurance company or if you have a vision rider. If you have a vision rider, we will need to know the name of the plan.

What if I think I have a medical problem?

We can file your insurance according to your chief complaint or the primary reason you are seeing Dr. Shettle. If you are having a problem with your eyes, you need to make sure you tell the doctor so that we can most accurately bill for your eye exam.

How do I request a copy of my medical records?

You may request your medical records either by mail or faxing a written request or release of information to our office. Please indicate the name of the institution or physician requesting the information along with the address, telephone and fax number and what reports you would like us to send. Attorneys, insurance companies and the State of Florida will be charged for medical records. There is no charge for patient or physician requests.


Payment Information

Dr. Lee Shettle accepts many popular insurance plans for exams and procedures he performs. Patients are able to pay using their major credit card.

Any co-payments or deductibles will be required to be paid at the time of your exam. We accept cash, check and major credit cards.

Financing Options

Care CreditWe are pleased to offer to our patients as a service the CareCredit card. This is North America’s leading patient payment program. CareCredit offers a full range of payment plans to meet every financing need of the patient. There may be no interest charges if the balance is paid in full within the predetermined time period. There are also lower monthly payment plans available to allow you more time to pay. There are no up front costs, no annual fees, and no pre-payment penalties. It is easy to apply and you will usually receive an online decision in a few minutes. Please ask our staff for more details.

You can get a “No Interest” plan every time you use CareCredit. Under the No Interest Plans for 6, 12, 18 or 24 months, each month you only need to pay the required minimum monthly payment and pay off the remaining balance within the promotional period to avoid interest and pay only the amount financed. Future minimum payments will vary based on amount and timing of payments, interest rate and other charges added to the account.

If you need more time to pay for your procedure, you can take advantage of our extended payment plans for 24, 36, 48 or 60 months with low interest rates.
